This section describes the record layouts and fields of for the data that you need to submit.  The file format and naming conventions that should be followed are also given.

Individual records should occupy one line in the file; each record ends with the line feed and carriage return characters.  Fixed width records will not be accepted.

Name your files according to the guidelines in the file naming appendix.  At a minimum the file name should begin with the correct form prefix File Naming and end with the ".txt" extension.

The widths specified in the data dictionary are the maximum allowable widths for the fields.
